If you are a parent with children ages 17 and younger, please fill out this survey about how you get your children to medical and health-related appointments.
If you are pregnant, please fill out this survey to let us know how you are getting to your medical and health-related appointments.
We want to know if you are having challenges getting to those appointments and what those challenges are.
This survey is part of a project with the Wayne County Health Department, and we hope it will help us find solutions to your transportation problems.
